New Membership Application

Become part of the Credit Union by first reviewing the Membership Disclosures and then completing the online Membership Application below, or by printing and returning it to the Credit Union office with either an initial deposit of $5.00 or by electing a Payroll Deduction on the membership application. A new account packet will be mailed to you with the required signature card to sign and return to us.

A Debit Card provides account access 24/7. You can access funds from your savings and/or checking when used at an ATM, or you can make point of sale (POS)* purchases drawn off your Electronic Checking Account.

Online Access:

Access your account information 24/7 with online access through our website at www.dowellfcu.com. You will be automatically enrolled upon opening your membership, a generic password will be mailed to the address on your account. This temporary password will allow you to login and customize your password.

eStatements:

All periodic account statements will be sent to you via secure e-mail to the e-mail address you provided above. If you do not provide an e-mail address, a paper statement will be mailed. E-statements can be saved to your files, printed, or viewed with Adobe Acrobat Reader. The password to open/view your e-statement will be the primary owners Social Security Number (with dashes--i.e. 555-55-5555).

(A) By signing below, I certify under penalties of perjury that (1) the Taxpayer Identification Number (TIN) shown on this Membership Application Form is my correct TIN and I am not subject to backup withholding either because (a) I have not been notified by the Internal Revenue Service that I am subject to backup withholding as a result of a failure to report all interest or dividends or (b) the IRS has notified me that I am no longer subject to backup withholding.
